Transitioning from Hospital to Home

This presentation focuses on the changing needs of today’s older adults, particularly those needs that are unique to hospital discharge including a discussion of common issues that may arise, questions to ask prior to discharge to avoid re-hospitalization, and a brief overview of types of services available to provide support at home.
